Our small group of select University of Connecticut School of Medicine affiliated physicians will provide truly state-of-the-art attentive and individualized medical services, including:

Limited patient panels will allow for members to have a personal relationship with their physicians and will allow our physicians to deliver superior medical care.
Members can schedule same-day or next-day appointments via telephone, e-mail, fax or the internet. Member walk-in appointments are also welcome.
Your physician will make home visits when it is medically necessary.
Your physician will oversee all hospital care, whether it be local, across the country, or across the world.
State-of-the-art Electronic Medical Record. Your medical chart will be accessible to your physician 24/7/365 on Broad Band Network, creating seamless continuity of care-in our office or anywhere in the world.
As a member you will have confidential access to your medical record via a secure internet patient portal. You can check your laboratory values and other aspects of your chart from home at any time. Your physician will contact you about your blood work that day and explain the significant findings.
Your physician will provide you with an updated wallet size medical information card, including medical conditions, medications, allergies, base-line ECG and personalized laboratory values. This accurate medical information is invaluable in case of emergency.
Same-day, automated prescription refills, via the web, fax, e-mail or telephone.
Our physicians are on-staff at the three largest teaching hospitals in central Connecticut. As a member you will get personalized referrals to the best specialists.

Each member will receive unparalleled personal assistance with their medical needs from our exceptional medical and administrative staff. Facilitation of appointments to see specialists, test scheduling, vaccines, and insurance claims assistance will be provided by our staff.
We will help members arrange transportation to tests and doctors’ appointments, if a member needs assistance.
Our physicians practice evidence-based preventive internal medicine. This special professional arrangement allows our physicians to keep abreast of advancing medicine. Your personal physician will have the time and resources to study you personally and supply the exact medical care you need. Modern-day medicine requires time.
A Web-based health news letter will be available to members. Our physicians read daily on a variety of medical topics and will maintain an on-going dialogue with members regarding pertinent developments via state-of-the-art web-page links, e-mail, phone, fax, voice mail or in person.
Our office has been professionally designed to make you feel at home. In the rare event that a member needs to wait for their physician (e.g., walk-in patients), the experience will be nothing less than first-class.
We provide our members with free on-site valet parking.
Members will receive annual consultations in nutrition and exercise physiology. Additionally, member patients will receive information and recommendations for non-traditional medical services, such as chiropractics, acupuncture, herbal therapy, holistic therapy, mind-body programs, bio-feedback programs and health spas.
When necessary, our physicians will take care of elderly patients’ special needs via home visits and coordinate aspects of home health care.

* All professional services are provided by Personal Care Associates, LLC (the “Practice”). Many of our professional services are covered by traditional health insurance. The Practice participates with most major health plans and accepts reimbursement from health plans as payment in full for covered professional services. Noncovered professional services and other items that are not covered by health plans are provided by or arranged through Personal Care Physicians, LLC, which charges a membership fee for such non-covered items and services.
