Benefits and services provided through the At Home Medical Care and House Call Program:

• Medical home visits to reduce unnecessary hospitalizations and repeat hospital stays.
• Up-to-date screening tests to prevent future medical problems.
• Careful tracking of medication needs, including prescribing and refilling necessary medications.

Pricing and Fees
The cost to become a patient of the At Home Medical Care and House Call Program is $3,500 for an individual, paid on an annual basis. In addition, a Hospital Care Plan is offered to patients who select to be under the direct care of their Personal Physician, including daily medical visits, while in the hospital. The additional cost for the Hospital Care Plan is $1,000 for an individual.
