Your private physician will recommend, coordinate, and closely monitor any specialist care you receive. Confidential follow up visits will be conducted with your concierge physician, based on your schedule, to address medical issues and concierge treatment solutions. Concierge Medicine – Private Physician Retainer Pricing and fee structure:

Concierge Medicine ( Private Physician Retainer Plan)
The cost to become a patient at Personal Care Physicians, LLC  is $2,750, paid on an annual basis, or $4,750 annually for couples. In addition, a Hospital Care Plan is offered to patients who select to be under the direct care of their Personal Physician, including daily medical visits, while in the hospital. The additional cost for the Hospital Care Plan is $1,000 for an individual. Please note, highly complex medical conditions/situations will be priced accordingly.
